Twenty weeks. Halfway. The scan everyone circles on the calendar. And one of the most Googled pregnancy questions in existence: "What does my baby actually look like right now?"

Here's the honest, detailed answer — from someone who looks at 20-week babies on screen regularly, and still finds them extraordinary every time.

Your Baby at 20 Weeks: The Real Picture

Size and Weight

Your baby at 20 weeks weighs approximately 10 ounces — about the weight of a small banana, which also happens to be close to their length from crown to toe at around 10 inches. They're big enough now that the anatomy scan can evaluate every major organ system in clear detail, and there's still enough amniotic fluid around them that the images are sharp and spacious.

What You'll Actually See on Screen

A profile that looks unmistakably human. Forehead, nose, lips, chin. The curve of a cheek. An eye socket. A mouth that may open and close while you watch. The features your baby will have on the day they're born are already visible at 20 weeks — and already distinctive. Many parents describe it as the first time they understood that there is a specific person in there.

What's Developing Right Now

  • Brain: The cerebral hemispheres are clearly visible. The brain is actively developing folds — gyri and sulci — that are measurable and assessable on ultrasound.

  • Heart: All four chambers are beating and visible. The anatomy scan evaluates the heart's structure more thoroughly than any other routine prenatal test.

  • Spine: Each vertebra is individually visible. The sonographer checks the spine from multiple angles.

  • Face: Lips, nose, palate (checking for cleft formation), eye spacing, jaw profile — all assessed.

  • Abdomen: Stomach, kidneys, bladder, abdominal wall, and cord insertion site are all examined.

  • Limbs: All four, including fingers and toes when positioned well.

  • Skin: Still somewhat translucent at 20 weeks — which is why 2D images look different from the rounded features you'll see in a 3D session at 28–32 weeks.

💡 What catches parents off guard: Most people expect a still, quietly posed baby. Real 20-week scans are dynamic. Your baby may be somersaulting, pressing their face into the placenta, tucking their chin, kicking, or waving during the entire appointment. Sonographers are trained to work around the movement — and sometimes the movement gives us our best views.

What the Anatomy Scan Actually Checks

The 20-week anatomy scan is a systematic medical evaluation that typically takes 45–60 minutes. Here's exactly what gets checked:

  • Brain structures: Lateral ventricles (size), cerebellum (structure and position), midline structures.

  • Face: Lips and palate, nose, jaw profile, eye spacing.

  • Spine: Checked from both transverse and sagittal views, vertebra by vertebra.

  • Heart: Four-chamber view, outflow tracts, major vessels.

  • Abdomen: Stomach, kidneys, bladder, umbilical cord insertion, abdominal wall integrity.

  • Limbs: All four extremities, long bone measurements.

  • Growth measurements: BPD, HC, AC, FL — all plotted against your gestational age curve.

  • Placenta: Location, appearance, and whether it's covering the cervix.

  • Amniotic fluid: AFI measured to confirm adequate fluid levels.

The Gender Question at 20 Weeks

Yes — the 20-week anatomy scan is when most families find out whether they're having a boy or a girl. Accuracy at this gestational age is very high (95%+) when baby is cooperative with positioning.

You have options: find out at the scan, ask for a sealed envelope for a private or public reveal later, or ask the team not to tell you at all. A good sonographer follows your lead — just let them know your preference at the start of the appointment.

📍 At Treasured Memories Imaging: We offer Gender Reveal sessions starting at 15 weeks ($89+). If you want the reveal experience in our studio — with the full session, the real moment, and the reaction captured — we do that beautifully. Gender can also be revealed in a sealed envelope for your own private reveal.

If Something Is Found at the Anatomy Scan

Most anatomy scans confirm a normally developing baby measuring appropriately for dates. Occasionally, a finding — an organ that looks slightly different, a measurement outside the expected range, or a 'soft marker' — requires follow-up.

Before you spiral: the vast majority of findings at the anatomy scan either resolve on their own or turn out to be normal variations. Ask your sonographer to explain any finding in plain language before you leave the room. You are entitled to a real explanation, not just a follow-up referral. Most things that prompt a second look turn out to be nothing worth worrying about.

How to Prepare for the Best Possible Anatomy Scan

  • No full bladder required — at 20 weeks, baby is easily seen without it. Just stay well-hydrated.

  • Eat a light meal or snack beforehand. A fed baby is often an active baby, which helps sonographers get all the views they need.

  • Wear a two-piece outfit. You'll be on the table for 45–60 minutes — comfort matters.

  • Bring your partner or support person. This appointment generates big emotions.

  • Write down your questions the night before. The excitement of the moment erases every carefully rehearsed question.

  • Allow extra time. Anatomy scans that run 70–80 minutes are not unusual when baby is active or in an awkward position.
